Welcome to Flagler Cares, the one-stop health and social care organization in Flagler County. Flagler Cares accepts clients who need support, direct assistance, and referrals to resources that can help keep them healthy and safe.
Our dedicated staff work with clients to identify needs and create solutions that address benefits, financial stability, food insecurity, healthcare access and insurance, mental health, substance use disorder, and other every-day challenges.
Flagler Cares offers a support system through a wide range of collaborative community partners to provide high-quality services, including the Flagler Village where clients can access multiple agencies in one location. Through their comprehensive knowledge, staff connect with clients to navigate the systems of support.
Flagler Cares offers an expanded Behavioral Health Program to serve those who are uninsured or underinsured in Flagler County. This program includes psychiatric evaluations, medication management, counseling, pharmacy assistance, and care coordination.
The services for this program take place at the Flagler County Village and consist of a telehealth visit as well as a visit with an in-person care coordinator. This ensures that technology does not become a barrier for services. Appointments for psychiatric services are set within three to five days which is significantly faster than the current norm.
Flagler Cares also supports access to free or low-cost medications through a Prescription Assistance Program in partnership with A Plus Pharmacy. The program is open to individuals with Medicaid coverage, individuals with very low income, and individuals without insurance or with very limited coverage.
As a part of our substance use disorder prevention efforts, Flagler Cares offers information on the safe use, storage, and disposal of unused prescription medications.
Flagler Cares has a team of certified Navigators who will assist eligible consumers with enrolling in health coverage plans. This program aims to provide consumers with educational information about the Affordable Care Act (ACA) and health insurance to make an informed choice based on unique and individual needs.
Our team of Navigators are trained and able to provide outreach and enrollment services in an unbiased way free of charge to anyone looking to enroll in health care coverage in the Federal Health Insurance Marketplace. We assist consumers with in-person, telephone, or virtual online enrollment services. Schedule an appointment with a Navigator near you or with a Flagler Cares certified Navigator.
