The Coordinated Opioid Recovery (CORe) initiative brings together multiple agencies working collaboratively to address the many needs individuals may face throughout recovery. While the primary focus is supporting those with opioid use disorder, services are also available for individuals experiencing other substance use disorders. Through a whole-person, multi-agency approach, clients have access to coordinated care, resources, and support designed to promote long-term recovery and stability. Our goal is to meet each person where they are and provide compassionate support every step of the way.

Through the Partnered for Success Program, staff help individuals and families build a path toward economic stability and long-term success. Participants partner with a coordinator to set goals, develop a plan, overcome barriers, and connect to services that support financial and health stability. This partnership is built on shared accountability, with ongoing support, regular check-ins, and a commitment to taking meaningful steps toward progress.

Flagler Cares staff can screen individuals for disability claim eligibility and either provide advice on the process or directly assist individuals through the process if they qualify for our SSI/SSDI Outreach, Access, and Recovery (SOAR) program.
